Client Rostering

We are pleased to share that we will roster unattached clients from your organization and connect them with a primary care provider if they meet any of the following criteria outlined below.

 

• Individuals who self-identify as Indigenous

• Individuals who are from racialized groups or communities

• Individuals with physical or mental disabilities, including mental health conditions and/or substance use challenges

• Individuals who identify as part of the 2SLGBTQ+ community

• Individuals experiencing homelessness

• Newcomers, including immigrants and refugees

• Individuals living in geographically or physically isolated areas who face barriers to accessing care

• Individuals living in conditions of extreme poverty
