The Peterborough Community Health Centre (CHC) is committed to improving access to primary care and wrap-around health services for people in our community who face barriers to care.
To support fair and equitable access, we prioritize people who do not have a family doctor or nurse practitioner and who are part of the priority populations that experience systemic barriers to healthcare.
How to Access Care
1. Visit the Peterborough Connect Clinic
If you do not currently have a family doctor or nurse practitioner, the Peterborough Family Health Team’s (PFHT) Connect Clinic can help support you while you wait to be connected to permanent primary care.
The Connect Clinic can help you:

Register with Health Care Connect

Navigate the intake process
-
Access primary care services while waiting for attachment
-
Receive care for acute, chronic, and preventative health needs

Access mental health navigation support
-
Build a medical record that can be transferred to your future primary care provider
The Connect Clinic works closely with local healthcare organizations, including the Peterborough Community Health Centre, to help connect patients with a family doctor or nurse practitioner when space becomes available.

2. Indigenous Support and Navigation
If you are Indigenous and need support accessing primary care, please contact the Indigenous Cultural Wellness Coordinator at the Peterborough Community Health Centre.
Support may include:

Connection to cultural wellness and community supports

Discussion about immediate wellbeing and support needs

3. Register with Health Care Connect
Health Care Connect (HCC) is a provincial program that helps people without a family doctor or nurse practitioner connect to primary care providers in their community.
The Peterborough Community Health Centre works with Health Care Connect to identify individuals who may be eligible for attachment to our primary care team when space becomes available. People are prioritized based on healthcare needs and other factors identified through the Health Care Connect process.

Call 8-1-1, available 24/7
Residents can register with Health Care Connect even if they are already on another local waitlist. Individuals who currently have a healthcare provider may also be eligible to switch to one closer to home through the program.
Priority Populations
As a Community Health Centre, our mandate is to serve people who do not have a primary care provider and experience barriers related to social, economic, cultural, or systemic factors.
When selecting new clients from the Health Care Connect waitlist, we prioritize individuals who identify with one or more of the following groups:

Individuals who self-identify as Indigenous

Individuals who are from racialized groups or communities

Individuals with physical or mental disabilities, including mental health conditions and/or substance use challenges

Individuals who identify as part of the 2SLGBTQIA+ community
-
Individuals experiencing homelessness

Newcomers, including immigrants and refugees
-
Individuals living in geographically or physically isolated areas who face barriers to accessing care
-
Individuals living in conditions of extreme poverty
